Introduction: Congenital syphilis (CS) is considered a sentinel event of the quality of prenatal care and is responsible for unfavorable outcomes to the fetus / newborn. Objective (s): To evaluate the notification rate and to draw an epidemiological profile of cases of congenital syphilis in the state of Alagoas between 2008-2017. Methodology: Retrospective descriptive observational study from public domain data from the Department of Chronic Diseases and Sexually Transmitted Infections of the Ministry of Health. For trend analysis, a regression point regression model was used and the AAPC was calculated, with a confidence interval of 95%, a significance of 5%. Results: 3141 cases of CS were identified from 2008 to 2017 and 89.14% had a diagnosis of recent CS. Among the pregnant women, 49.12% were 20-29 years old, 84.72% were brown and 31.87% had incomplete elementary school. As for health care 72.59% of pregnant women had prenatal care. There was also a decreasing trend in postpartum diagnosis (AAPC = -5.4%, p <0.001). The treatment schedule of the pregnant woman was inadequate or not performed in 88.93% of the cases and the untreated / ignored partner in 91.38%. Regarding the child's age, 94.91% were diagnosed with less than 7 days. Conclusion: The findings highlight some weak points of syphilis care and prevention in Alagoas. The pursuit of syphilis eradication has been a challenge to health services, health authorities, as well as professionals and society at large.
